APPLICATION FOR USE OF THE JOHN GUMM BUILDING AND CIVIC CENTER

By submitting this application, the applicant and the organization the applicant represents accept and agree to abide by all of the County's regulations/policy (insert document here) regarding use of this facility; furthermore to indemnify, defend, and hold the County harmless from any claims or actions arising out of the group's acts or omissions pursuant to its use of the facilities. 

The county assumes no responsibility for loss, damage, or liability/injury that may arise from the use of the facilities, unless arising out of the acts or omissions of the county, its officers, employees, subcontractors, or agents. 

Reservations must be made at least 30 days prior to the event. Deposits, Insurance Certificates, and OLCC approvals (if applicable) are required at least 15 days prior to the event.
